A Matter of Balance

A Matter of Balance is designed to reduce the fear of falling and increase the participants' comfort in remaining active. Two trained instructors lead the two hour class that meets once a week for eight weeks. Goals include learning to stop the fear cycle, increase activity through gentle exercise, and increase comfort in remaining actively engaged in life. 

Cancer: Thriving and Surviving

The Cancer: Thriving and Surviving Program is a workshop given for two and a half hours, once a week, for six weeks. Workshops are facilitated by two trained leaders, one or both of whom are peers who are cancer survivors themselves. This curriculum is specifically written for cancer survivors. These classes are highly participatory, where mutual support and success build the participants’ confidence in their ability to manage their health and maintain and active fulfilling life. Topics covered include techniques to deal with problems such as frustration, fatigue, pain, isolation, poor sleep and living with uncertainty; appropriate exercise for regaining and maintaining flexibility and endurance; making decisions about treatment and complementary therapies; communicating effectively with family, friends and health professionals; nutrition; setting priorities; and relationships.  

Chronic Disease Self-Management Program

This is the workshop for adults with any type of an ongoing or long-term condition such as diabetes, pain, arthritis, cancer, high cholesterol, asthma, heart disease, or high blood pressure. This free workshop is highly interactive and meets for two and a half hours, once a week, for six weeks. Participants build new relationships and learn new skills to help them manage all the things needed to improve their health, as well as to help keep them active in their lives and relationships.  

Chronic Pain Self-Management Program

The Chronic Pain Self-Management Program is a workshop given two and a half hours, once a week, for six weeks. Workshops are free to attend and facilitated by two trained leaders. Subjects covered include techniques to deal with problems such as frustration, fatigue, isolation, and poor sleep; appropriate exercise for maintaining and improving strength, flexibility, and endurance; appropriate use of medications; communicating effectively with family, friends, and health professionals; nutrition; pacing activity and rest; and how to evaluate new treatments. Classes are highly participatory, where mutual support and success build the participant's confidence in their ability to manage their health and maintain active and fulfilling lives.  

Creaciones de la Cocina-Spanish version of Kitchen Creations

Taught in Spanish, this Kitchen Creations curriculum uses the Cocinando para Latinos con Diabetes cookbook instead of the 4-Ingredient Cookbook. Kitchen Creations is a program that teaches people with diabetes how to plan and prepare meals that will help manage blood sugars. Participants attend four classes, each about three hours long. They receive nutrition education from a Registered Dietitian or CDE and cook recipes in small groups to help them apply what they are learning. They enjoy the meal the class cooked and set goals for healthy lifestyle changes that work for them. Participants receive a manual and cookbooks to keep.

Moving for Better Balance

Tai Ji Quan: Moving for Better Balance® is a falls prevention program derived from a contemporary routine known as Simplified 24-Form Tai Ji Quan (pronounced tye gee chuwan). This class transforms martial arts movements into a therapeutic regimen aimed at improving postural stability, awareness and mindful control of body positioning in space, functional walking, movement symmetry and coordination, range of motion around the ankle and hip joints, and lower-extremity muscle strength.  

National Diabetes Prevention Program

The National Diabetes Program is an interactive, small group,lifestyle change program to help reduce the risk of Type 2 diabetes. The program consists of 16 group core sessions delivered over 24 weeks, followed by a post-core for the remainder of the year. Participants will reduce their risk of Type 2 diabetes by focusing on losing weight through healthy eating, being more physically active, and learning to recognize and overcome barriers to making healthy choices.  

Otago Exercise Program

The Otago Exercise Program for falls prevention is designed for those who may be frail and still living in the community. Otago is taught by Physical Therapists and Physical Therapist Assistants primarily in the home setting. The curriculum is delivered over 8 weeks with the participant doing the exercises independently between sessions. The Otago Exercise Program requires a scheduled assessment and session planning through a local provider. Otago is covered under Medicare and Medicaid. Call for more information and to schedule an assessment. 

Positive Self Management Program (HIV)

The Positive Self-Management Program is a course for participants managing a positive HIV condition. This free workshop is given two and a half hours, once a week, for six weeks, in community settings such as senior centers, churches, libraries and hospitals. Workshops are facilitated by two trained leaders. Subjects covered include: techniques to deal with problems such as frustration, fatigue, isolation, and poor sleep; appropriate exercise for maintaining and improving strength, flexibility, and endurance; appropriate use of medications; communicating effectively with family, friends, and health professionals; nutrition; pacing activity and rest; and how to evaluate new treatments. Classes are highly participatory, where mutual support and success build the participant's confidence in their ability to manage their health and maintain active and fulfilling lives.  

Programa de Manejo Personal de la Diabetes- Spanish DSMP

Taught entirely in Spanish, Programa de Manejo Personal de la Diabetes is for adults with type 2 diabetes and their caregivers. This free workshop is highly interactive and meets for two and one half hours, once a week, for six weeks. Participants build new relationships and learn new skills to help them manage life with type 2 diabetes. Topics include improving communication, healthy food choices, staying active, and maintaining good relationships.  

Stepping On

Building confidence and reducing falls, Stepping On is a small group program designed to reduce falls and build confidence in older adults. The program is a two-hour session once a week for seven weeks, followed by a booster session three months later. Participants engage in building self-confidence, making decisions and adding safety strategies to their everyday life. Guest experts lead segments in the areas of exercise, vision, medications and community safety. Stepping On covers a range of topics including fear of falling, strength and balance exercises, home hazards, safe footwear and safe clothing, vision as it relates to falls, community mobility, coping after a fall and understanding medication risks. 

Tai Chi for Arthritis and Falls Prevention

Gentle tai chi forms that focus on relieving the symptoms of arthritis are also shown to reduce the risk of falls. Easy and enjoyable to learn, this tai chi brings with it many health benefits safely and quickly. Proven to reduce pain, Tai Chi for Arthritis for Falls Prevention is a safe exercise that is ideal for beginners, with or without arthritis. The class meets for one hour, one to two times per week, for eight weeks.  

Tomando Control de su Salud- Spanish version of CDSMP

Tomando Control de su Salud (Chronic Disease Self-Management) is taught entirely in Spanish. This free workshop is highly interactive and meets for two and a half hours, once a week, for six weeks. This workshop is for adults with any type of an ongoing or long-term condition such as diabetes, pain, arthritis, cancer, high cholesterol, asthma, heart disease, or high blood pressure. These culturally competent classes teach skills and build success in the participants' confidence in their ability to manage their health and maintain an active and fulfilling life.  

Toolkit for Active Living

Learning how to manage your physical and emotional problems can be a challenge, but you can do it from your home. The Tool Kit introduces information and skills that are helping people with many different physical and mental health conditions lead a healthy life. The Toolkit includes 1) A Self-Test that will help you find where you want to focus. 2) Tip Sheets about specific topics that others have found key in managing their conditions. 3) Living a Healthy Life with Chronic Conditions the latest edition of a book written by experts in the field. 4) A relaxation CD with four different relaxation exercises. 5) An exercise CD with three different levels of exercises, all of which can be done at home.

UNM-H Prediabetes Class

Learn what you can do to avoid Diabetes. This 2 hour class is a requirement for anyone interested in taking the National Diabetes Prevention Program (NDPP) at University of New Mexico Hospital's Center for Diabetes & Nutrition Education. Must pre- register and have qualifying factors met. Course is free, open to the public.

Instructor Training in Matter of Balance

This one day Leader/Coach training prepares the participant to deliver a Matter of Balance. The program requires 2 Leaders/Coaches to lead the community class. Participants in the training will practice following the leader manual content and process; recognize that for older adults, the fear of falling is a significant risk factor in falling; use effective modeling strategies in leading the group exercises; and recognize how to handle people and situations appropriately. Class includes the Training Manual.  

Instructor Training in Moving for Better Balance

This two day, eight hour per day training will prepare instructors to teach Moving for Better Balance in the community. Tai Ji Quan: Moving for Better Balance® is a falls prevention program derived from a contemporary routine known as Simplified 24-Form Tai Ji Quan. Class includes the Training Manual.  

Instructor Training in Tai Chi Arthritis

Instructor training for Tai Chi for Arthritis for Falls Prevention is two days focusing on the basic six core movements and the advanced six extension movements. Instruction will cover the essential Tai Chi principles, precautions, warm up and Qigong (exercise to improve relaxation and vital energy). Successful participants in this training will receive a two year certification to teach the class. The certification is through Dr. Paul Lam and Tai Chi for Health. The class instructor is Master Trained by Dr. Paul Lam. This training includes an instructional DVD and manual.

This two day class trains the Chronic Disease Self-Management Program leader in the Cancer: Thriving and Surviving curriculum. To satisfactorily complete this leader training class, participants are required to complete all of the training activities and complete one practice teaching activity. Refresher training's offered to LSCT NDPP coaches- annually.  

Lifestyle Coach Training/NDPP

This is a two day training to become a Lifestyle Coach in order to offer the National Diabetes Prevention Program classes in the community. The class is an interactive, small group, lifestyle change program to help reduce the risk of Type 2 diabetes. Materials include the Lifestyle Coach training manual, the NDPP Participant manual, and a calorie count book.

Master Trainers are certified according to the national Self Management Resource Center guidelines. Candidates be certified leaders and complete training in a specific curriculum in order to train workshop leaders.
