Local classes

All classes listed below are proven, evidence-based programs that have been shown to help people better manage their conditions. Please click on them for more detailed information. To find a class near you, click on the “Find a Class” button.

Costs for classes vary by type of class and provider. Some are free, some are covered by Medicare and might require a co-pay, and some have a fee. Check with the provider of the class for cost details.


Classes for Prevention of Diabetes

Diabetes Prevention Program (DPP)
The Diabetes Prevention Program is a year long program for small groups of people who are at risk for developing diabetes. Classes are led by a trained lifestyle coach. People with take the classes learn real changes such as healthier eating, adding physical activity into their daily lives, and improving problem-solving skills. Classes meet weekly for the first 16 weeks, then less often for the rest of the year. This program can help people lower their risk of getting type 2 diabetes by 58%.


Classes for People With Diabetes

Diabetes Self-Management Education (DSME)

10-hour program for people with type 1, type 2, or gestational diabetes. Family members are also welcome. This class helps participants take care of their diabetes day-to-day. Classes are led by licensed or certified health professionals.

Diabetes Self-Management Program (DSMP or Living Well with Diabetes)
A 6-week program for people with type 2 diabetes. Caregivers who assist them are also welcome. The class helps people to adopt and maintain healthy behaviors and to live active and fulfilling lives. Classes are led by a team of peer leaders.
Programa de Manejo Personal de la Diabetes

A 6-week program for Spanish-speaking people with type 2 diabetes. Caregivers who assist them are also welcome. The class helps people to adopt and maintain healthy behaviors and to live active and fulfilling lives. Classes are led by a team of peer leaders.



Living Well Classes for People With Other Chronic Conditions

Chronic Disease Self-Management Program (CDSMP or Living Well)
A 6-week program for people with any chronic diseases or conditions that go on for a long time. Caregivers who assist them are also welcome. The class helps people to adopt and maintain healthy behaviors and to live active and fulfilling lives. Classes are led by a team of peer leaders.

High Blood Pressure
A 2½ hour class that is offered before the 6-week CDSMP or DSMP class. This class helps people diagnosed with hypertension or high blood pressure to better manage their condition.

Chronic Pain Self-Management Program (CPSMP or Living Well - Pain)
A 6 week program for people with chronic pain. Caregivers who assist them are also welcome. The class helps people to manage their lives with pain and stay active. Classes are led by a team of peer leaders.

Cancer: Thriving and Surviving (CTS)
A 6 week program for people who are cancer survivors. Caregivers who assist them are also welcome. The class helps people to adopt and maintain healthy behaviors and to live active and fulfilling lives. Classes are led by a team of peer leaders.
